To qualify for these Limited Warranties you must have your puppy examined by a licensed Veterinarian (of your own choosing) within SEVEN days of purchase or within the time span afforded you by the regulations of the municipality in which the puppy is purchased. You must also keep the puppy under regular and continued care of a licensed Veterinarian for the full term of these Limited Warranties.

If, in the opinion of your veterinarian, your puppy has any physical problem which makes it a poor heath risk, you may, within fourteen days of purchase, return the puppy to this establishment for a full refund or full credit towards another one of our puppies of your choice.

To make a claim under this provision, bring your puppy back to this establishment as quickly as possible after your veterinarian's examination. You must also bring a signed statement from the veterinarian showing the date, within the fourteen-day period, on which she/he conducted examination and the reason she/he believes the puppy to be a poor health risk. This statement must include a description of the symptoms supporting his/her opinion.

Since it is your intention to return the puppy for a refund or credit, your veterinarian should not provide: any further immunization or treatment for the puppy. Be assured that all necessary care will be provided alter consultation with our veterinarian upon the puppy's return to this establishment.

If, within one year from purchase date, your puppy is diagnosed by your veterinarian as having a debilitating hereditary defect that in his/her opinion prevents your puppy from serving as your companion pet, this establishment will, at your option, provide you with either a full refund credit towards another one of our puppies of your choice.

To make a claim under this provision, bring a signed statement from your veterinarian stating the date of his/her diagnosis, the nature of the defect, his/her statement that the defect is hereditary in nature, and the reasons why the defect prevents the puppy from serving as a companion pet.

At the opinion of this establishment; you may also be required to secure a second veterinarian's opinion, at our expense. In some instances, we may also require you to return puppy. In all instances, you will be required to return all registration documents for the puppy.

If you should lose your puppy due to illness (accidents are not included) within two years from the date of purchase, this establishment will credit your original purchase price towards another one of our puppies of your choice.

To qualify under this provision, you must keep the puppy under the regular and continuous care of a licensed veterinarian. This care must include a complete immunization and worming program recommended by your veterinarian, immediate treatment for any obvious signs of illness or injury and routine annual physical examination.

To make a claim under this provision, you must bring a veterinarian's certificate showing the date and cause of loss to this establishment as quickly as possible after the this has occurred. You must also authorize release to our veterinarian, a complete copy of your puppy's health care records for the period of ownership.

The Following terms and conditions apply to these limited warranties.

A.   You qualify for a refund or credit for another puppy only for the reasons and in the manner stated herein.

B.   If you are eligible for and select a refund under these warranties, this establishment will refund your puppy's full purchase price by company check within 14 days of receipt of all documentation substantiating your claim.

C.   If you are eligible for and select credit to be used towards another puppy, you may take up to six months to choose that replacement at this establishment.

D.   If you choose another puppy from this establishment, which has a greater retail price than the amount of your credit, you must pay the difference. If you're new choice has a retail price less than your credit you may use the difference for other purchases or, at your option, the difference will be refunded to you by company check within 14 days of the transaction.

E.   This establishment provides puppies solely for the purpose of a companion pet. There is no warranty expressed or implied that you will be able to use this puppy for showing or breeding purposes. You should not purchase a puppy from this establishment if either of these uses are a condition of your purchase.

F.    The warranties concerning health, life, and condition are limited as expressed. This establishment is not responsible for any payments of veterinarian fees for examinations, drugs, vaccinations, x-rays, laboratory testing, or registration application documents for this that have been delivered to you. You must also assign any and all documents for this puppy back to this establishment before a refund credit will be issued to you.

G.   If you are eligible for and select a refund or credit under the provisions of this warranty, you must return any registration or registration application documents for this puppy that have been delivered to you. You must also assign any and all documents back to this establishment before a refund or credit will be issued to you.

H.   This establishment is individually owned and operated. Any claim under the provisions of these Limited Warranties must be made only to this establishment at the address listed.

I.   These Limited Warranties are given in lieu of all other warranties expressed or implied. Purchaser agrees that there are no other remedies or warranties which extend beyond the preceding terms and conditions.

 This written agreement is the entire agreement between me and this establishment concerning the health, condition, development, use or loss of the puppy purchased today. I will not rely on any oral statements contrary to this written agreement unless they are included in writing. I have read and completely understand these warranties, including the General Terms and Conditions, all of which I accept.

This initial program of protection and treatment was designed and administered in consultation with Dr. John Lionberger D.V.M. Additionally, a complete medical record for this puppy has been maintained while under our care and will be made available to you during the next 12 month period if you so request. We have counseled your client about the need for a continuing program of veterinary care, to both fill their responsibility as a pet owner and to validate this warranty.

While every effort is made to see that no puppy leaves this store if showing clinical symptoms of illness, your examination may uncover a problem that was not evident to our veterinarian or client prior to sale. Should this situation arise, we would appreciate your call before treatment in order to determine whether refund or replacement is more satisfactory alternative for the ins.
